Gepirone (Exxua): Heart Rhythm Checks, Warnings and Side Effects

Gepirone, sold as Exxua, is an extended-release tablet FDA-approved for major depressive disorder in adults. Its label requires heart rhythm tests called ECGs and carries a boxed warning about suicidal thoughts in young people. Our psychiatrist sees people online in Pennsylvania and New York, so ask us what an ECG would involve.

Key points

  • The FDA label approves gepirone for major depressive disorder in adults.
  • It can prolong the QT interval, a measure of heart rhythm, so the label calls for ECGs before and during treatment.
  • Its boxed warning covers suicidal thoughts and behaviors in children, teens and young adults.
  • It must not be used with MAOIs or strong CYP3A4 inhibitors, or by people with severe liver impairment or long QT syndrome.
  • An ECG cannot be done over video, so this is a medicine where in-person testing is part of the picture.

What gepirone is

Gepirone is a prescription antidepressant and the active ingredient in Exxua. It comes as an extended-release tablet. MedlinePlus groups it with the serotonin modulators, medicines that work mainly on serotonin, a natural substance in the brain.

What makes this page different from most antidepressant pages is the testing. Gepirone comes with specific heart rhythm requirements, explained below. This is general information from the FDA label and MedlinePlus, not advice about your treatment. Our list of medicines covers others.

What it is approved for

The current FDA label lists one approved use: the treatment of major depressive disorder (MDD) in adults. It is not approved for children or teens, and the label says it is not approved for treating bipolar depression.

This page describes only the label's approved use.

How it is thought to work

The label says the way gepirone helps depression is not fully understood. It is thought to relate to changing serotonin activity in the brain through selective action at one serotonin receptor, called 5HT1A.

What taking it can be like

The label says gepirone should be taken with food at about the same time each day, and swallowed whole without splitting, crushing or chewing. MedlinePlus says it may take several weeks to feel the full benefit, and that your doctor may adjust treatment based on how it works for you and any side effects.

Testing is a regular part of it. According to the label, the steps around treatment include:

  • An ECG before treatment begins. The label says not to begin if the baseline QT measure is too long.
  • More ECGs while the dose is being adjusted, and periodically during treatment.
  • Correcting electrolyte problems, such as low potassium or magnesium, before treatment, and further electrolyte checks for people at risk or on diuretics or glucocorticoids.
  • More frequent ECGs for people taking other medicines that prolong QT, or at significant risk of a dangerous heart rhythm.
  • Telling your prescriber about every other medicine, since some raise gepirone levels in the body and others lower them.

Boxed warning and who should not take it

The boxed warning is about suicidal thoughts and behaviors. Antidepressants increased this risk in children, teens and young adults in short-term studies. Everyone taking an antidepressant should be closely watched for worsening depression and new suicidal thoughts, and families can help by watching for changes too.

The label says gepirone must not be used by people with a known allergy to it, a long QT measure at baseline, congenital long QT syndrome, or severe liver impairment. It also must not be used by people taking strong CYP3A4 inhibitors, or by anyone taking an MAOI or within 14 days of stopping an MAOI or gepirone, because of the risk of serious and possibly fatal interactions. The label also says not to begin it in someone being treated with linezolid or intravenous methylene blue.

Side effects and other warnings

The most common side effects in the label are dizziness, nausea, trouble sleeping, stomach pain and indigestion. MedlinePlus also lists headache, sleepiness, dry mouth and others, and says to call your doctor if any are severe or do not go away.

Other serious warnings in the label are QT prolongation; serotonin syndrome, with more risk alongside SSRIs or tricyclic antidepressants; and mania or hypomania, which is why the label says to screen for a history of bipolar disorder and its risk factors. MedlinePlus says to call your doctor right away or get emergency treatment for seizures, or for fever, sweating, confusion, fast or irregular heartbeat, severe muscle stiffness or twitching, or hallucinations.

MedlinePlus says to continue gepirone even if you feel well, and not to stop it without talking to your doctor. The National Institute of Mental Health (NIMH) gives the same general advice about prescribed medicines. Tell your prescriber if you are pregnant, plan to become pregnant or are breastfeeding.

Can it be managed online in Pennsylvania and New York

Partly, and it depends on testing. A secure video visit can cover your symptoms, history and other medicines. An ECG or a blood test for electrolytes cannot be done over video, so anyone taking gepirone would need those arranged in person, at a place that can share the results with the prescriber.

A licensed psychiatrist on our team, a Doctor of Osteopathic Medicine (DO), treats depression by secure video for people physically located in Pennsylvania or New York during each visit. Whether it is an option for you is a decision for you and your prescriber. Ask us whether our psychiatrist can help. See online depression treatment, psychiatry in Pennsylvania or send us a medication question.

Our therapists do not prescribe. Their depression therapy is by video in Pennsylvania, West Virginia, Arizona, Colorado and Florida, not New York, so New York readers can get psychiatry here but not therapy here. There are many ways to get support with depression, and starting a conversation is a good first one.

Common questions

What is an ECG and why does gepirone need one?

An ECG records the heart's electrical activity. Gepirone can prolong the QT interval, a part of that recording, and a long QT can be linked to a dangerous heart rhythm called torsade de pointes. The label calls for an ECG before treatment, while the dose is being adjusted, and periodically during treatment.

Can other medicines affect gepirone?

Yes. The label rules out strong CYP3A4 inhibitors, calls for a dose change with moderate ones, and says to avoid strong CYP3A4 inducers, which lower its levels. MAOIs are ruled out, and SSRIs or tricyclic antidepressants raise serotonin syndrome risk. Medicines that prolong QT call for more frequent ECGs. Give your prescriber a full list.

Is gepirone used for anxiety?

The label lists only major depressive disorder in adults as an approved use. This page does not describe it as an off-label treatment for anxiety. If you are looking at medicines for anxiety, your prescriber can talk through what is approved and what fits your history.

Can your psychiatrist prescribe gepirone online?

We cannot say a visit will lead to any particular medicine, and gepirone needs in-person ECGs in any case. Our psychiatrist treats depression by secure video for people physically located in Pennsylvania or New York during each visit. Whether it is an option for you is a decision for you and your prescriber. Ask us whether our psychiatrist can help.
