Website Privacy Policy

This policy covers the website locwellness.com. Information about your care as a client (your health record) is protected separately under HIPAA and is described in our Notice of Privacy Practices.

What we collect and why

• Contact form. Your name, an email address or phone number, the state you are in and how you prefer to be reached. If you choose to tell us, also who the care is for, whether you plan to use insurance, and the best time to reach you. We use it only to reply to you. The form has no message box on purpose: please do not send health details through the website.
• How you found us. Your browser remembers, in its own storage and not in a cookie, the page you first arrived on, the website or ad that sent you and the last few pages you viewed. It is sent to us only if you submit the contact form, together with your approximate city and state and the type of device, so we know which of our outreach is reaching people. It is kept for up to 90 days and you can clear it in your browser settings.
• Online booking. Before you book, we ask for your name, email address and phone number so we can follow up about your booking. You then choose a time on Zocdoc, our booking partner. What you enter on Zocdoc goes to Zocdoc and to us under Zocdoc’s own privacy policy, not to this website. Once you are a client, scheduling and intake are handled in SimplePractice, our secure client portal.
• Analytics. When analytics is switched on, we use Google Analytics to count visits and see which pages are helpful, including whether a booking, phone or email button was clicked. We do not send your name, contact details or anything you type into a form to analytics.
• Server logs. Our website host records standard technical data (IP address, browser, pages requested) to keep the site secure and running.

What we do not do

• We do not sell or rent your information.
• We do not use session-recording tools or advertising pixels that follow you around the web.
• We do not add you to a marketing list because you contacted us.

How contact form messages reach us

Contact form messages are delivered to the practice by ordinary email through an email delivery service. A copy goes to our website provider, whose only role is to check that messages are arriving. Email is not a secure channel for health information, which is why we ask you to keep clinical details for the secure client portal or a session.

Cookies

The site itself does not need cookies to work. Google Analytics, when on, sets cookies to tell visits apart. You can block them in your browser settings and the site will work the same.

Links to other sites

We link to Zocdoc, SimplePractice, state licensing boards and crisis services. Each has its own privacy policy.

Your choices

You can ask what information we hold from your website inquiry, or ask us to delete it, by calling 267-573-9817 or emailing info@locwellness.com.

Changes

If this policy changes, the new version will be posted here with a new effective date.
