Notice of Privacy Practices

Your health information is yours. This page explains your rights under HIPAA and how to get a copy of our full Notice.

Locascio Wellness is required by law to keep your health information private, to give you a Notice of Privacy Practices that describes how your information may be used and shared, and to follow the terms of that Notice. You receive the full Notice when you become a client. You can ask for a copy at any time by calling 267-573-9817 or emailing info@locwellness.com.

Your rights

When it comes to your health information, you have the right to:

  • get a copy of your health record;
  • ask us to correct your record if you think it is wrong or incomplete;
  • ask us to contact you in a specific way, or at a specific address or number;
  • ask us to limit what we use or share, including not sharing with your insurer when you pay for a service in full yourself;
  • get a list of those with whom we have shared your information;
  • get a copy of the Notice of Privacy Practices;
  • choose someone to act for you;
  • file a complaint if you feel your rights have been violated.

How your information is typically used

Health care providers generally use and share health information to treat you, to run the practice, and to bill for services. Therapy notes get extra protection, and most other uses need your written permission. There are legal exceptions to confidentiality, including a risk of serious harm to you or someone else and suspected abuse or neglect of a child or vulnerable adult.

If you have a concern

Talk to us first if you can: call 267-573-9817. You can also file a complaint with the U.S. Department of Health and Human Services Office for Civil Rights at hhs.gov/hipaa/filing-a-complaint. We will never retaliate against you for filing a complaint.
